This Billing & Financial Policy is provided to help you understand our fees, payment expectations, and how superbills work. Our goal is transparency—so there are no surprises.
Important: Everwell Counseling Center is a self-pay practice. We do not bill insurance directly.
Upon request, we can provide a superbill that you may submit to your insurance provider for possible out-of-network reimbursement. Reimbursement is determined solely by your insurance plan and is not guaranteed.
1. Self-Pay Practice and Superbills
We are an out-of-network, self-pay counseling practice. Payment is collected directly from the client. We do not verify benefits, obtain prior authorizations, or submit claims to insurance on your behalf.
If you would like to seek reimbursement, you may request a superbill. A superbill typically includes service dates, fees, provider information, and common billing codes your insurer may request.
You are responsible for understanding your insurance coverage, including out-of-network benefits, deductibles, copays/coinsurance, reimbursement rates, and claim submission requirements.
2. Good Faith Estimate and the No Surprises Act
If you are uninsured or not using insurance (including choosing not to submit claims), you may be entitled to receive a Good Faith Estimate of expected charges for scheduled services, as described under the federal No Surprises Act.
A Good Faith Estimate is an estimate—not a contract—and actual charges may vary based on clinical needs, changes in scheduling, or services requested.

3. Fees and Payment Due
Our fees for services are provided upon request and may be shared during scheduling or intake. Payment is due at the time of service unless a written arrangement is made in advance.
Accepted payment methods may include credit/debit card and other available methods communicated by our office.
4. Late Cancellations and No-Shows
We reserve appointment times specifically for you. Late cancellations and no-shows may be subject to a fee. If applicable, our cancellation window and fees will be provided during intake or scheduling.
Insurance companies typically do not reimburse cancellation or no-show fees, and the client is responsible for these charges.
5. Returned Payments, Chargebacks, and Past Due Balances
If a payment is declined, returned, or disputed (chargeback), you remain responsible for the balance due. We may require an alternative payment method to continue services.
Accounts with unpaid balances may be subject to additional steps consistent with applicable laws and professional guidelines.
6. Refunds
Refunds are generally not provided for completed services. If you believe an error was made in billing, please contact us promptly so we can review and resolve the issue.
7. Billing Questions and Itemized Receipts
You may request an itemized receipt for your records. If you need documentation for reimbursement or an HSA/FSA, please let us know and we will provide the appropriate receipt when available.
For superbills, please confirm your insurer’s requirements, including deadlines and whether you must submit claims electronically or by mail.
